In Work Package 6 PRIME-CKD aims to translate the research findings into concrete clinical practice recommendations to foster implementation of personalized medicine in clinical practice. To achieve this aim PRIME-CKD has established a stakeholder network to assist in the translation of research findings and implementation in clinical practice.
